Preliminary Health Questions

This is a confusing time, but we have decided to keep our mask requirement and other protocols in place since we work on clients for prolonged periods of time and because the Covid-19 situation keeps changing and now we are dealing with Monkeypox as well…

**As of August 2022, I have not gotten Covid-19.  I still practice a high level of safety when in public, in the office, and in my home.

Look over the questions and simply TEXT your responses.  Example:  1 yes 2 no 3 no 4 no:

(1)  We are unusual in that our service puts us VERY CLOSE to all clients.  Are you ok with wearing a face mask when you are  face up on the table (we utilize a different face cradle system that allows clients to remove their masks when face down). __ yes __ no

(2)  Are you immune-compromised or do you have anyone living in your home who has a compromised immune system?  __ yes  __ no

(3)  Are you a FIRST RESPONDER (ambulance worker, nurse or doctor caring for Covid-19 patients, police officer, etc.)? __ yes   __ no

(4)  Have you experienced any symptoms of Covid-19 in the last two weeks (cough, shortness of breath, difficulty breathing, chills, fever, or loss of taste or smell)?  __ yes  __ no

(5)  Are you an essential worker who works around other workers who have tested positive for Covid-19 (Amazon, Nestle, Walmart, grocery stores, etc.)? __ yes   __ no

(6)  Have you been around anyone who tested positive for Covid-19 in the last two weeks?  __ yes  __ no

(7)  Have you experienced any fever, malaise or swollen lymph nodes in the last few days?  __ yes  __ no

(8)   Do you or have you had in the last week, any undiagnosed blisters, pustules or scabs that started as blisters anywhere on your body?  __ yes  __ no
